On this page you will find useful documents including new patient forms, links to helpful websites, and information on books that are often used in therapy. If you are just starting services at Serenity Psychological Services and Consulting LLC you may complete the forms below prior to your appointment to reduce paperwork time during your visit. You will need to complete either the adult or youth intake packet. You may or may not want to complete an authorization to request or release information depending on your circumstances, and we will discuss this at your first appointment.

Health care providers and other agencies may fill out the referral form below and email it to medrecords@serenitypsc.com or fax it to (866) 473-6903 to refer an individual for services. Our email and fax are HIPAA compliant.
